Here’s how to use it:
- Download the form and save it to your computer.
- Type your company name at the top field
- Type your instructions in the bottom box
- Save this new version on your computer
- Send to clients via email or have staff use this for in-office visits!
Remember to enter the fax number or address that you want people to return the form to. Because you need a real signature, people won’t be able to completely fill this out online (unless they have a registered e-signature). But by allowing them to type in the rest of the fields, the form will be much easier to read than if they hand write it.
