Here’s how to use it:

1. Download the form and save it to your computer.
2. Type your company name at the top field
3. Type your instructions in the bottom box
4. Save this new version on your computer
5. Send to clients via email or have staff use this for in-office visits!

Remember to enter the fax number or address that you want people to return the form to. Because you need a real signature, people won’t be able to completely fill this out online (unless they have a registered e-signature). But by allowing them to type in the rest of the fields, the form will be much easier to read than if they hand write it.
